CREW LEADER ENTRY FORM


  1. THE CREW LEADER REGISTRATION FORM MUST BE COMPLETED BY THE CREW LEADER OF EACH CREW (ONE PER CREW), WHICH INCLUDES ALL CREW MEMBER NAMES AND DATES OF BIRTH. REGISTRATION FEES FOR THE ENTIRE CREW MUST BE PAID IN ONE PAYMENT BY CREW LEADER. INVOICE WILL BE SENT TO CREW LEADERS WITHIN 3-4 BUSINESS DAYS.

  2. EACH CREW MEMBER MUST COMPLETE THEIR OWN WAIVER FORM.

IMPORTANT! PLEASE READ CAREFULLY

CREW MEMBERS - All crew members submitted on the official registration form must be competing.


SUBSTITUTE DANCERS - Substitute dancers will become a crew member only if and only when justifiable cause necessitates a crew member to be substituted and HHI NZ approves such substitution in advance. *Do not register substitute dancers or alternative dancers.


ENTRY CHANGES, ADDITIONS, SUBSTITUTIONS - Crew member substitutions or additions once the NZ HIP HOP DANCE CHAMPIONSHIP has begun are not permitted except for reasonable and justifiable cause (For example, injury, hospitalisation, illness under a doctor's care and/or death). A substitution/addition of a crew member(s) within the crew may not be made without prior notice and an approval received from HHI NZ.


CREW MEMBER SUSPENSION/DISQUALIFICATION - Falsification of personal information and/or of national eligibility documents by a crew, crew member, or representative and/or the substitution/addition of a crew member(s) without approval are subject to disqualification, suspension and/or other severe penalties deemed necessary by HHI NZ. Crew or crew members who are in breach of the code of conduct may be subject to disqualification, suspension and/or other penalties deemed necessary by HHI NZ. Code of Conduct can be found on page 24 of the Official Rules and Regulations Manual.
